But seriously, with the difference in projectile diameter, shouldn’t there have been some sign that something was wrong before firing?

I think, as with all things in life, it depends.

Shape and length of the .30 cal projectile matters for sure. Some can chamber in a 5.56 easier than others. From reading the article my take was the owner of this rifle had a friend shooting it when he grabbed the wrong mag. Perhaps that friend wasn't as versed in the platform and didn't know enough to say "hey that felt funny" when he dropped the bolt on a bomb.

And it is mostly ^ that reason that I paint my .300BLK rifles/pistols and mags to match. Camo mag, camo gun. Black mag, Black gun.

I have teenagers and less experienced adults shooting my rifles often.
 
This is why I exclusively use Lancer mags for 300BLK and PMAGs for .223.

The pics of the bullet after being fire formed to .223 are very cool!

Once again Mr Stoners invention did it's job of containing the carnage. Several times I have seen this and several times nobody lost any fingers or eyes. I'm sure they lost their bowel control.

Nothing a little JB weld won't fix. Also, there's a Wilson Combat 5.56 barrel out there for sale somewhere, barely used, less than a hundred rounds thru it :D

I'm surprised they were able to push it out with a cleaning rod.
