3rd gen 4Runners, for some reason or another, are lacking one of these above the driver's door. You can feel the bolt holes there, underneath the headliner.
Made a handle from cordage, liked it and replaced the other three handles.
4Runners, while I love them, are hell for headroom. I have no idea how Lurch (@Tailhunter ) manages to get into his without busting his noggin. Anyhow, I've had the blessing of being in the passenger seat when the truck hit a pretty solid bump and striking your head on the factory grab handle smarts a bit. They seem to be sturdy (they've had 190 lbs hanging from them and as an added bonus, they're much softer than the factory handles.
