purchase permit in Randolph co?

What is the process now for getting a pistol purchase permit in Randolph? I looked at the sheriffs website but I'm not understanding what all they want. It used to be just walk in, fill out the papers and 10 minutes later leave with your permit. Now I have no idea what I'm supposed to do.
 
What is the mental release form? This part is new to me. Print it, fill it out ( I assume I fill it out, does a dr need to fill it out?), get it notarized (will any notary do or...?)
 
You will be signing, and having notarized, a document which permits “them” to research your mental condition. You’re allowing the collection of this information which THEY will collect.
