QD sling mount failure

Has anyone had any issues with their sling releasing from a rotation limited QD mount? I have a quality QD sling that has started to pull out of one of my better QD mounts with limited use.

This only happens when a lateral force is applied to the mount. When pulling straight out of the mount it is secure. On other rifles I am not seeing this issue.

After looking around some this appears to be a common problem and wanting to know if you guys have had any issues with running them hard.
 
I had this issue once before. I had back to back flush cups on a Magpul stock for sling use on either side. The screw that fastened the flush cups together thru the bottom was just slightly too long. The sling QD would fasten into either side and hold quite well but a strong tug would pop it out. I filed a thread length or too off the screw and now it works perfectly. The protruding screw was just long enough to rub the finish off the end of my QD swivel, that is how I figured it out.

This may not be any help but it is the only issue I have ever had. I am sure BFG will take care of you.

Company is sending out a new mlok QD mount today. They said there have been only 2 accounts, mine and one other, in the past year of their mounts failing.
