Along with the hemorrhagic fevers especially Ebola, plagues, and other bioboogers, small pox is the one that got my attention most when I was learning about and teaching NBC stuff.

 
Do not forget tuberculosis. Today it is fairly easy to detect and diagnose (usually by X-ray then culture) but while treatment is more successful it ain’t that easy … time and lots of specific antibiotics. The US is pretty lucky in the TB department … very few active cases today but say 100 or so back it was a 50-50 chance of death and even those who survived didn’t have it easy and faced a fair chance of recurrence.

Anyway … TB especially in less developed areas and countries was never brought under real control and flares up occasionally … in fact Native American reservations were 5 times more likely to have a TB response from the CDC than any other area in the US. Even worse are many South American and Caribbean countries who never went thru the TB vaccine phase like the US started in the first half of the 1900’s which helped most of the US clear out TB. Today, with the influx of illegals from way more countries than just Mexico no telling how many carry TB and when it could rear up again.

My son got 'pre-TB' when he was in grade school. When we asked the doc how this was possible he said that it was as easy as sitting next to an illegal on a bus.

Smallpox was supposed to be contained in Atlanta and in Russia, but it's common knowledge that other actors could have it. Weaponizing ebola or other hemorrhagic fevers is incredibly hard, weaponizing smallpox is actually pretty easy.

No joke … Remember back in Desert Storm Saddam Hussien was trying to weaponized Camelpox because he thought most Iraqi people would be immune since they were around camels for life while foreign troops would not be immune. All it takes it one wacko with enough money to “buy” a couple scientists and a lab and presto changeo we got smallpox running though younger generations and even some older people who’s smallpox vaccination from when we were kids timing out possibly.

I would imagine China and NK at least.

Most animals have a pox, most are inconsequential to people. And smallpox is only natural in people, but some USAMRIID docs figured a way to make it transmit to monkeys. But the essential components are similar, and it wouldn't be hard to weaponize it and modify smallpox.

Only in the military and high risk people.
I got it some years ago along with some others in anticipation of working in Asia. I just asked my doc for them, no evidence of travel or anything else required.
