i've heard about index pins getting sheared and notches getting deformed... but yowzers. thanks for sharing.
 
Don't know what a reaction rod is but I’m guessing it engages the bbl lugs like the bolt would to prevent what happened from happening. If that’s correct then it should have been locked into the vice not the upper/lower. A teaching moment.
 
The reaction rod is supposed to engage the lugs and prevent torquing the upper and/or shearing the pin, which is often the weak point.

With a clamshell forces go Muzzle Device >> Barrel >> Barrel Extension >> Pin >> Upper >> Clamshell >> Vise
With a reaction rod it goes Muzzle Device >> Barrel >> Barrel Extension >> Reaction Rod >> Vise

Proper use of the rod wouldn't have changed anything because the weak point here was the barrel Barrel <> Extension threads

The rod I’m using has a spline that fits into the charging handle race. So, in my case, the forces were….

Muzzle > barrel > extension (failure point) > ROD > upper > vise

In any case, as I acknowledged above and several have noted, I misused the rod.
 
This ended badly, but kudos to you for showing us HOW it happened.

A learning experience shared.

The barrel turned in the extension. IOW, the extension didn’t move at all while the barrel was backed out of the extension.
 
So the bottom line is that if you want to remove a muzzle device and don’t know how tight it really is as you didn't install it you need to at least remove the handguard so that the bbl can be safely put in a vice not the upper/lower or reaction rod.
