Guy I teach Sunday School with got this pic as a quiz to see if he could identify it. Neither of us has any idea.

The only other clues he got was “M200” and these two pics:

I've often wondered how many of those have been shot off the ends of guns.

In my experience, about one per each year of my BCT drill experience. So about 1 in a thousand or so.

One happened due to the ASP mixing live ammo and blanks somehow and the cadre not checking the blanks prior to the BCT soldiers loading it up. On one hand, I understand because it’s SUPPOSED to be blanks. On the other hand, cadre is responsible for EVERYTHING so they should’ve, just in case. And unfortunately it was on an M240B/ 7.62mm machine gun, not a SAW or M4/16
 
Concur MILES (laser tag) for training. Blank Firing Adaptors for the M16 (2x small red square boxes) and M60 (3x rectangle orange ones). M200 refers I'm assuming to the nomenclature of the 5.56mm blank round.
