The barrel extension should be torqued to about 150ftlbs, far more than should ever be needed to remove a muzzle device. I also think that some are pinned. As you saw, lots of bad things will happen before you get to 150ftlbs.
 
Also, wouldn't a left hand thread be more likely to loosen the barrel under firing due to rifling twist? While the current setup actually tightens the barrel?

Or do I have that backwards?
 
I would recommend 100 ftlbs of torque along with pinning the extension. Before that clean all threads well and use red locktite. The extension is intended to be permanent once the gas port drilling and all other fitting is complete.
