Are there any attorneys present on the forum who create NFA Trusts?

What are the resources available other than a kiosk to get fingerprints done?

Once you have a finger print card and photo for a responsible person, is it simply scanned and attached to the correct eForm?

Yes…you’ll simply upload photos. You can either upload an .eft fingerprint file (not something you can create yourself) or mail them in.

Photos are also DIY, so don’t pay for actual passport photos.
